Request my subsidy certificate via e-demarches
You live in the canton of Geneva and can no longer find your subsidy certificate? Go to your e-demarches account to download your attestation online.
If you don't have an e-démarches account, register directly online www.ge.ch

Request my subsidy certificate by post
If you don't have an e-démarches account, you can request a copy of your subsidy certificate by post from the SAM (Service de l'Assurance Maladie). Please note that you will be charged CHF 20 for this.
What are the steps involved in requesting my attestation by post?
- 1. Send a written request to:

1207 Genève - 2. A payment slip will be sent to you, mentioning my CHF 20 to pay (or CHF 50 for a family)
- 3. Once proof of payment has been received, you will receive the requested certificate at your home address.
